The DPPO plan allows you to visit any dentist without referrals for all your dental care. You may seek dental care from any provider; however, your out-of-pocket expenses will be lower if care is provided by a dentist in the Cigna network. Out-of-network providers may charge you the difference between what they charge and what your insurance pays.

The DHMO provides a higher level of benefits and lower out-of-pocket costs than the DPPO plan. There are no deductibles, benefit maximums, or claim forms. However, you are required to choose a Cigna primary care dentist for all of your dental care and referrals. If you do not use your primary care dentist for all of your services and referrals, the plan does not pay any benefits.

Note: DHMO plans are not available in the following states/territories: Alaska, Maine, Montana, New Hampshire, New Mexico, North Dakota, South Dakota, Vermont, Wyoming, Puerto Rico, US Virgin Islands, and Guam. If a Team Member and/or their dependent lives in these states or territories, the DPPO is the only available option.

2. Click on the Find Care & Costs tab at the top of the screen.
3. Enter your location and search by type of provider, name of provider, or reason for visit.
4. Read reviews and find office details on the next screen before choosing the best provider for you.

Semi-annual cleanings help to prevent the development of more serious health issues and lower your out-of-pocket dental expenses. If you are covered under either of the Shake Shack Dental plans, preventive care is at no cost to you when you see an in-network dentist!
